General inquiries
The contact form collects your name, reply email, a general inquiry category, and acknowledgment of this notice. It has no message box, medical questionnaire, or attachment upload. Netlify processes and stores submissions and screens them for spam. Email notifications are configured to a privately managed mailbox. Site administrators and authorized mailbox users can access inquiries to respond. A successful form submission does not confirm that an email notification was delivered.
Do not include symptoms, diagnoses, medical history, records, or other sensitive health information in any field. This form is for general inquiries, not clinical intake or urgent care. Ask the clinical team for an appropriate way to share health information. Submitting an inquiry does not book an appointment or establish a doctor–patient relationship.
Contact information is used to handle your inquiry. The form does not subscribe you to marketing. Inquiry records may be retained in Netlify and in notification mailboxes. No automatic deletion schedule is implemented by this website. For questions about retained information, or to request access, correction, or deletion, contact the team through the general inquiry form without adding sensitive health details.
Optional Google Analytics
Analytics is off unless you choose to allow it. If you opt in, Google Analytics processes technical visit information, such as browser and device details, to measure site use. The site configures analytics cookies with a 90-day expiry and stores your analytics choice in your browser for up to 180 days. If browser storage is unavailable, your choice applies to the current page.
The site’s analytics configuration disables advertising storage, advertising personalization, and Google signals. The page-view event uses a generic site address and title without condition-related page paths, URL queries, fragments, or referrers. The site counts the start of a form interaction and a successful form response using only a fixed form identifier. These optional events contain no inquiry fields or health details and respect the same analytics choice and browser privacy signals. After opt-in, this same limited page measurement applies on every page, including the contact form. Automatic form-interaction measurement is disabled in the Google Analytics stream to avoid duplicate or unconfirmed submission counts.
Google’s account-level measurement and retention settings also affect its processing. See Google’s privacy policy. Browser privacy signals such as Global Privacy Control or Do Not Track keep analytics off. Use Analytics settings in the footer to change your choice. Turning analytics off clears this site’s analytics cookies and reloads the page to stop the loaded tag.
Hosting and browser features
Netlify hosts this website and may process technical request information, including network and browser information, to deliver the site, maintain security, and prevent spam. See Netlify’s privacy policy. Fonts and images are served with the site. The navigation and resource filters run in your browser. This site has no visitor accounts or embedded social-media feeds.
